4Reelz School of Film Terms & Conditions

All students, staff, and / or any person(s)(the “Participant(s)”) attending or participating in 4Reelz School of Film and any related activities must agree to the following terms and conditions, as indicated, in its complete and entire form. If under the age of 18, a Parent or Legal Guardian must provide consent for Participant.

By agreeing to the Terms and Conditions Participant confirms their desire to participate in the programs and activities (the “Program”) relating to enrollment and / or participation in 4Reelz School of Film. Participant understands that in order to participate in the Program, whether as a paid or unpaid participant, he/she must agree to be bound by this General Consent, Talent Release, Medical Consent, Release of Liability, Waiver of All Claims and Assumption of Risk Agreement’s (“Release Agreement”) in favor of 4Reelz School of Film, its directors, employees, heirs, executors, licensees, successors, and assigns (the “Producer”) and all the Releases (as defined below).

General Consent and Talent Release

The Participant , and on behalf of any heirs, successors and assigns, hereby irrevocably and perpetually agree and grant Producer:

Participant's consent and the right to photograph participant, (candidly or otherwise and alone or with others) or otherwise reproduce participants likeness or image and to record and reproduce participants voice, whether on film, tape, disc or otherwise, in connection with the Program, as well as, all advertising and publicity relating thereto and all transcripts and derivative versions thereof.
Participant's consent and the right to project, broadcast and exhibit in any and all media of any and every nature whatsoever now known or hereafter devised, said photographs, likenesses, images, and recordings and other reproductions of my voice, in whole or in part, in connection with the Program (and all versions, adaptations and trailers thereof) and all advertising and publicity relating thereto and all transcripts and derivative versions thereof.
Participant's consent and the right to quote from, paraphrase, edit, re-write, and add material to and/or delete material from and otherwise make use of said photographs, likenesses, images, and recordings and other reproductions of Participants voice.
Participant's consent and the right to copyright (in the United States and elsewhere) for the initial and any renewed or extended period of copyright, in 4Reelz School of Film’s name or any such name as Producer may elect, to any works derived from my participation in the Program, including but not limited to any and all screenplays, versions, adaptations film works and trailers thereof. All advertising and publicity relating thereto, all transcripts and derivative versions thereof and all said photographs, likenesses, images, and recordings and other reproductions of Participant voice.
Participant hereby agrees that the Producer may exercise all or any of the rights herein granted by Participant without claims, demands or causes of action at law or in equity, whether for libel, defamation, violation of right or privacy, or infringement of any literary or other property right or otherwise.
Participant hereby agrees to release Producer from and against any and all claims and/or disputes which Participant may have for invasion of privacy, right of publicity, defamation, or any other cause of action arising out of the production, distribution, broadcast, webcast or exhibition of any kind from Participants participation in the Program.

Discipline and Behavior Management Policy

The Participant , and on behalf of any heirs, successors and assigns, hereby irrevocably and perpetually agrees as follows:

Particpant will abide by the following Basic Class Rules:

Gentle hands and feet
Use listening ears while others are speaking (staff and fellow campers)
Use friendly words with everyone
Respect all property
Clean up after oneself

To maintain safety and an overall pleasant environment 4Reelz School of Film responds to behaviors and provides preventative strategies throughout our time together. We promote a positive behavior support with the use of reminding language, redirecting language, warnings regarding personal choice options, timeout to reconsider a poor choice, and follow up for making better choices in the future. We support problem solving and will guide children in this endeavor.

However, if after the use of the above strategies and environment support, Participant continues to choose disrespectful and/or unsafe choices, (harmful to oneself or others), 4Reelz School of Film will continue to the following steps, in an effort to maintain a safe, enjoyable atmosphere for ALL our Students.

Depending upon the severity of the situation, the following possibilities will be considered:

Call to parents to either offer supportive language over the phone or immediate pickup if necessary
Dismissal from the program with a refund for remaining scheduled course
Dismissal from the program without a refund

Participant agrees to the following with regards to behavior and discipline:

I have reviewed, or will review School Rules with my child, prior to attendance and as requested throughout participation in any program.
I am willing to support 4Reelz School of Film in their endeavor to maintain a safe, pleasant environment for ALL Students and staff.
I will be on the ready to respond if my child has made poor choices by picking them up immediately if necessary.

Medical Authorization, Release & Consent

Participant declares, for myself and on behalf of my heirs, successors and assigns, understand that Producer cannot be expected to control all of the risk articulated in this form and that Producer may need to respond to accidents and potential emergency situations. Therefore, Participant hereby gives consent for any medical treatment that may be required, as determined by a medical professional, during participation in the Program and agrees to indemnify and hold harmless Producer for any such costs incurred to treat Participant, even if Producer has signed hospital documentation promising to pay for the treatment due to Participant inability to sign the documentation.

Participant further agree to release, waive, discharge, covenant not to sue, and agree to hold harmless for any and all purposes, Producer from any and all liabilities, claims, demands,injuries (including death), or damage including court costs and attorney’s fees and expenses. Participant understands this waiver does not apply to injuries caused by intentional or grossly negligent conduct.

Release of Liability, Waiver of All Claims, and Assumption of Risk Agreement

ASSUMPTION OF RISK; PARTICIPANT IS AWARE THAT PARTICIPATING IN THE PROGRAM AND ITS RELATED ACTIVITIES CAN BE POTENTIALLY HAZARDOUS TO PARTICIPANT AND THAT PARTICIPANT COULD BE SERIOUSLY INJURED. PARTICIPANT IS ELECTING TO PARTICIPATE IN PROGRAM ACTIVITIES WITH KNOWLEDGE OF THE DANGER INVOLVED, AND AGREES TO ASSUME ANY AND ALL RISKS OF BODILY INJURY, DEATH OR PROPERTY DAMAGE, WHETHER THOSE RISKS ARE KNOWN OR UNKNOWN.

RELEASE OF CLAIMS. Participant hereby releases and forever discharges Producer from any and all actions, claims, demands, causes of actions, damages, costs, liabilities, losses and expenses of every kind and nature whatsoever, whether known, or unknown, foreseen or unforeseen that Participant, and Participant's assignees, heirs, distributes, guardians, next of kin, spouse and legal representatives now have, or may have in the future, for injury, death or property damage, related to (i) participation in the Program, (ii) the negligence or other acts, whether directly connected to Producer or not, and however caused, by any Releasee in connection with the Program and all related Activities and (iii) the condition of the premises where these Program Activities occur, whether or not Participant is participating in these Program Activities. Participant also agrees that Participant, and Participant's assignees, heirs, distributes, guardians, next of kin, spouse, family members, executor and legal representatives will not make a claim against, sue or attach the property of any Releasees in connection with any of the matters covered by the foregoing release.

SEVERABILITY. If a court or arbitrator of competent jurisdiction holds any term of this Release Agreement invalid or unenforceable, such term will be reduced or otherwise modified by such court or arbitrator to the minimum extent necessary to make it valid and enforceable. If such term cannot be so modified, it will be severed and the remaining terms of this Release Agreement will be interpreted in such ways as to give maximum validity and enforceability to this Release Agreement.
